Please your form request by emailing [email protected]. Requests will be processed within 5 business days and may be picked up from the BIG Rewards Desk, mailed to you, or emailed to the address associated with your account. Please include the following information in your email:
- Full Name
- The Form You Are Requesting (Win/Loss or W2-G)
- Street Address
- Big Rewards Card Number
- Phone Number
- Tax Year Requested
- Date Of Birth

I hereby request that BJ’s Bingo & Gaming furnish me with the requested W-2G/WIN-LOSS documentation for my personal use. I hereby hold BJ’s Bingo & Gaming harmless of any errors that may be contained within the requested document(s). I also understand that the figures contained within the requested document may not be indicative of my complete gaming win and/or loss and are only that which is recorded while utilizing my personal BJ’s Bingo & Gaming BIG Rewards card. By executing this request, I verify that I am the primary account holder. I understand that falsifying my identity is a fraudulent act and is punishable by law. BJ’s Bingo & Gaming is not responsible for the accuracy of this estimate. Consult your tax advisor if you have any questions regarding deducting gaming losses on your tax return. Please note: Requests will be processed within 5 business days. The request may be picked up from the BIG Rewards Desk, mailed, or emailed to the address on file. I also acknowledge that previous years’ W-2G reprints may be subject to a service fee, which I am willing to pay.
